Security Guard Killed After Gate Falls on Her at Burrtec Industries Facility

On Saturday, May 25, 2013, at about 5:15 PM, San Bernardino Police and Fire personnel were called to location in the 5400 block of Industrial Parkway, in north San Bernardino. On arrival they found that a security guard had been crushed by a heavy steel security gate that had fallen off its track. The guard, Maria Luisa Peinado, a 64 year old resident of Colton, was pronounced dead at the scene. The security guard was crushed to death when she tried to put a heavy steel security gate back on its track and it fell on her. Peinado, a Colton resident, was pronounced dead at the scene. Police and the state Division of Occupational Safety and Health are investigating her death.

Salvation Army worker gets arm caught in conveyor belt

A worker at a Pasadena Salvation Army facility suffered major injuries to his arm Wednesday after getting the limb caught in a conveyor belt.

The workplace mishap was reported about 11:45 a.m. at the Salvation Army, 35 Waverly Drive.

Eleven firefighters and paramedics arrived at the scene within about three minutes and set to work with specialized equipment to free the worker’s arm. It took at least a half an hour dismantle the machine.

Rescuers rendered medical care and provided the man pain medication as they worked. He was described as being in his 30s.

The victim suffered major trauma to his arm and was taken to an area hospital.

It was not clear if the arm would need to be amputated.

The conveyor belt the man became caught in is used to sort donated clothing in a Salvation Army warehouse.

Representatives of the facility could not be reached for comment Wednesday afternoon.

It was not immediately clear how the accident occurred.

The California Department of Industrial Relations Division of Occupational Safety and Health will conduct an investigation.

Worker killed by lion was female volunteer

A female volunteer worker at Project Survival’s Cat Haven at Dunlap is dead in an attack by an African lion, a state Department of Fish and Wildlife official said Wednesday.

Lt. Tony Spada of state Fish & Wildlife confirmed that one worker was killed by the lion in an enclosure at the haven, at 38257 E. Kings Canyon Road. The haven is permitted by the agency for exotic animals.

 According to Cat Haven founder and executive director Dale Anderson the 26-year-old intern was attacked and killed when she entered the lion’s enclosure, , but he refused to provide more details.

Investigators were trying to determine why the intern was inside the enclosure and what might have provoked the attack, sheriff’s Sgt. Greg Collins said. The facility is normally closed on Wednesdays, and only one other worker was there when the mauling happened, he said.

The male African lion, a 4-year-old male named Cous Cous, had been raised at Cat Haven since it was a cub, said Tanya Osegueda, a spokeswoman for Project Survival, the nonprofit that operates the animal park.

Another park worker had unsuccessfully tried luring the lion into a separate pen, so deputies shot and killed it so they could reach the wounded woman, who died at the scene, Miller said.

Anderson did not field questions after talking to reporters. He did not say why the victim had entered the lion’s enclosure.

Anderson said Project Survival would investigate to see if the intern and the other worker on-site followed the group’s protocols.

Spada described this type of incident as “very rare” because of the safety measures required by the state and because regulations require minimal human-animal interaction.

The attack occurred about 12:30 p.m. when the worker was inside an enclosure with an African lion named Cous Cous, The Fresno County Sheriff’s Office said.

Another employee made several attempts to distract the lion away from the victim and into another enclosure, to no avail.

The business was closed to the public after the attack.

Spada said his agency will investigate along with the Sheriff’s Office to pin down what happened, including an autopsy of the lion.

 

Fresno County Emergency Medical Services got a call around 12:32 p.m. to respond to the Cat Haven. But before their arrival at 12:52 p.m., the call was canceled because the patient had died.

An autopsy on the worker will be conducted Thursday, Fresno County Coroner David Hadden said. The worker’s name and hometown were not released.

Osegueda, the Cat Haven spokeswoman, said Cous Cous was 4 years old and had been raised at the sanctuary since it was an 8-week-old cub.

Drowning deaths: Civic body slaps Rs 50,000 fine on contractor

A day after three brothers drowned in a pit dug along the Mithi river in Kalina, the Brihanmumbai Municipal Corporation (BMC) fined the contractor a meagre Rs 50,000. The civic body also sent a showcause notice to Shyam Narayan on Wednesday, asking him to explain the security lapses around the construction site in terms of proper barricading. BMC officials admitted that the security guards employed by the contractor were not present at the site on Tuesday since it was a public holiday.

While the Kurla police station had earlier named MMRDA officials in the FIR on Tuesday, on Wednesday it was made clear by MMRDA that the work was being done by the BMC. “Investigations showed that the BMC contractor is responsible for the negligence that claimed the lives of three children. We have now named the contractor in the probe,” said Deputy Commissioner of Police (Zone V) Dhananjay Kulkarni.

According to senior civic officials, contractors are responsible for maintaining security around construction sites. They are expected to adhere to the tenders conditions, whereby they are expected to adequately barricade the construction site and deploy security guards to monitor the area. Often, however, the tin barricades are stolen by local residents who sell them in the scrap market.

Chief engineer (storm water drains) L S Watkar said, “We have taken the necessary action by penalising the contractor. He has been asked to respond to the security lapses.”

“Anyone can easily trespass to the trench after crossing the premises of Kailas Parbat residential building at Kapadia Nagar. People from our slum have been regularly complaining to all agencies and the police department to put in place security measures since most of the children play here for lack of any other place,” said Azhar Chaudhury, resident of the area and uncle of the three children who drowned on Tuesday.

Eight-year-old twins Abdul Rahim and Abdul Karim and 12-year-old Abdul Aziz were playing in the open area adjacent to the river at Kapadia Nagar on CST Road when the incident took place. The pit was dug as part of the ongoing Mithi river project being undertaken by the BMC, under which retaining walls are being built to train the flow of the river.

17 injured, several critically, in two-alarm fire at Stag Hotel in Watsonville

 

WATSONVILLE – Seventeen people were injured, five critically, in a two-alarm blaze Monday at the Stag Hotel downtown, authorities said.
At least one man jumped from a second-floor window as flames tore through the 85-year-old two-story residential hotel at 117 W. Beach St. about 5:45 p.m. Thick smoke filled the street as dozens of onlookers watched firefighters from several agencies tackle the fire.
Ambulances and helicopters were called out for multiple casualties. Four of the injured were taken to Watsonville Community Hospital, four to Dominican Hospital in Santa Cruz and three others were airlifted to out-of-county trauma centers, Watsonville Fire spokeswoman Rosa Meyer said.
Several other residents were being treated in the parking lot behind the hotel for what appeared to be smoke inhalation, though paramedics also were putting a splint on the leg of a man laying on the ground.
The cause of the fire is under investigation. The extent of the damage is still to be determined.
“My initial thought was hopefully everybody got out safely,” said Ernest Araiza, who has owned the hotel for about nine years. “Buildings can be fixed or repaired. Lives are precious.”
A shoeless resident who gave his name as Barefoot Timmy said he heard the fire alarm go off and opened the door to a hallway filled with smoke. He retreated and climbed out his first-floor window.
Next door, at the Discount Mall, store owners worried the blaze might spread so they closed up shop and evacuated along with their customers. The mall’s janitor, Victor Mendoza, rushed over with two ladders to help second-story residents get out.
“I was walking down the hall and suddenly I heard, ‘Fire, fire, bring ladders,'” Mendoza said. “There were big flames coming out the front window. I just did what I could.”
Ben Puente was driving by, saw the fire and pulled over. He said he ran along the building yelling for people to get out, and saw one man jump from a second-story window and land on the ground. He helped others down the ladders.
Manager Russ Rickman said he had walked to the Subway shop two blocks away to pick up dinner and returned minutes later to see flames shooting from a window of a first-floor room at the front of the building.
“It happened so quickly,” he said.
Firefighters from the station a block away on Second Street responded quickly, Rickman said. Fire Chief Mark Bisbee said they immediately went to work rescuing residents and doused the flames in 8-10 minutes.
Rickman said the hotel has 50 rooms – three apartments in two buildings. He said about 45 men lived there. The fire was in the larger front building.
The building, constructed in 1927, had no sprinklers. The residents are a mix, some disabled, some struggling with substance abuse or in recovery, some on parole. All are men with little income.
Bisbee said the building was close to the top of his list of worries when he assumed leadership of the department in 2004. Though likely built to the standards of its time, it doesn’t meet codes required of current construction, he said.
“If there’s any silver lining, when it’s rebuilt, they’ll have to install sprinklers,” Bisbee said.
The Santa Cruz County Chapter of the American Red Cross set up an emergency center around the corner at Marinovich Park on Second Street. Sondra Ziegler, coordinator of the Disaster Action Team, said residents were being fed soup and hot drinks and pizza was on the way about 8 p.m. She said the Red Cross would put the displaced in motel rooms, most likely for the next three days while living arrangements are sorted out.

Neighbors Say Rotting Staircase Killed SF Woman

SAN FRANCISCO (CBS 5) — The San Francisco District Attorney’s office has launched a formal investigation into conditions at an upscale Nob Hill apartment building where a woman recently fell to her death through an apparent rotted staircase.

“That (death) is pretty shocking,” Ed Sweeney, an inspector with the San Francisco Building & Inspection Department, told CBS 5. His agency is also conducting a probe into the circumstances surrounding the fall that claimed the life last weekend of 62-year-old Rory Moore.

Moore was walking down the outside steps from her apartment in the four-story building when she fell through the staircase, fracturing her ribs and lacerating her liver and kidneys; she later died from internal bleeding.

Residents in the building at 1430 Taylor Street, where rents average from $3,000 to $4,000 a month, said that Moore was the third person in as many years to fall through the steps — which appeared to be suffering from dry rot. The tenants contend the rotting staircase was never replaced despite their complaints about the prior falls; the other victims survived.

The management company for the Lembi family, which owns the building, refused to comment on the allegations of negligence. Inspectors indicated the landlord has been cited previously for numerous code violations at their properties in the city and have been he subject of lawsuits.

“Multiple cases against the Lembi family are still pending,” Sweeney said.

Concord woman dies from injuries in Monday apartment fire

 

 

CONCORD — A woman seriously injured in an apartment fire earlier this week has died, authorities said.

Elise English, 34, of Concord, was taken off life support Wednesday morning after spending the past two days in the hospital, said Contra Costa Fire Marshal Lewis Broschard III.

English was found unconscious and not breathing in her unit at the Clayton Creek Apartments on Clayton Road on Monday after firefighters got a 1:38 p.m. call to an intense two-alarm fire. Firefighters performed CPR and she was taken to the hospital.

Firefighters extinguished the fire, which consumed English’s apartment and the roof, within about half an hour. No one else — including English’s roommate — was injured, and the fire damaged a half-dozen neighboring units.

It was the second fire to occur at the apartment complex that month: On Jan. 6, a one-alarm fire sent a resident to the hospital for smoke inhalation.

Woman in Wheelchair Killed in San Bernardino Fire

SAN BERNARDINO, Calif. (KTLA) — An arson task force was on the scene of a deadly fire in San Bernardino Monday.

Debriah Millsap, 58, was found dead inside her apartment after firefighters extinguished the flames.

The blaze was reported around 11:15 p.m. Sunday in the 800 block of G Street.

Police officers arrived to find the house fully engulfed in flames.Firefighters managed to pry open the security door and could see the Millsap inside, but the smoke and flames kept rescuers from reaching her.Millsap had multiple medical issues and used a wheelchair, according to Deputy Coroner Lisa DeNardo.

The official cause of death has not been determined.

Man crushed between wall and trash bin in La Jolla alley

SAN DIEGO (CNS) – A man in his 70s or 80s was found dead, pinned between a commercial trash bin and a wall in La Jolla Wednesday. An employee at Jonathan’s Market in the 7600 block of Fay Avenue found the body about 7 a.m., according to San Diego police. His name was not immediately available. Police looked at video from the store and saw images of a tractor-trailer backing into the alley, leading them to believe the truck hit the garbage bin and pushed it into the man, San Diego police spokeswoman Lt. Andra Brown said. It isn’t clear if the driver knew what he had done. Police said they believe the driver was trying to make a delivery to Jonathan’s, unaware that the receiving dock had closed hours earlier. Authorities have not released his name, but some employees at Jonathan’s told police they recognized the victim as someone who lived nearby, police Sgt. Jim Reschke said.

Customer injured in Stanton doughnut shop crash

A doughnut shop customer was critically injured when a car crashed into the business Tuesday morning. According to Capt. Marc Stone of the Orange County Fire Authority, the crash was reported at 9:50 a.m. near the intersection of Western and Katella avenues.

Lori Wertz, 53, was turning into the parking lot of the business and continued to drive into the shop, said Lt. Roland Chacon of the Orange County Sheriff’s Department.

Why Wertz, a New Mexico resident, was unable to stop was not immediately known, he said.

A customer inside the shop suffered a large cut to his right forearm, he said.

He was taken to UCI Medical Center in Orange to be treated, Stone said.

The injuries are not believed to be life-threatening.

The driver was shaken, but not injured in the crash, said Lt. Roland Chacon of the Orange County Sheriff’s Department.

How the crash occurred is still under investigation, but authorities said a driver crashed her vehicle through the business, sending glass from the window toward a customer in the shop. Deputies are investigating the crash.
