Traffic Piles Up for Miles After Fatal Crash on 405

LOS ANGELES, Calif. (KTLA) — A three-car wreck Tuesday night on the 405 Freeway left one person dead, California Highway Patrol officials said.
The crash happened about 8:30 p.m. on the southbound 405 just north of the Santa Monica Boulevard off-ramp. Details of the crash have not yet been released, but aerial photography provided by KTLA’s Sky5 suggest the wreck included two sedans and an SUV.
One person has been confirmed dead at the scene, CHP officials said.
Los Angeles city fire officials said no one else was injured in the crash and no one was taken to hospitals for treatment.
CHP officers have closed most of the southbound 405 Freeway, only leaving the southbound carpool lane open to motorists. The lane closures resulted in a traffic nightmare, with cars inching along for miles behind the wreck.
Northbound lanes have not been closed but traffic has also slowed significantly.
CHP officials expect the southbound lanes to remain closed for several hours.
Cause of the Los Angeles County Crash

Fatal Crash on Interstate 15 leaves 2 Dead
Southbound Interstate 15 has reopened in Victorville after the freeway was closed for more than 2½ hours by a collision that killed two people.
A wrong-way driver caused the head-on collision that closed the interstate’s southbound lanes near Palmdale Road, the California Highway Patrol reported. It was earlier indicated that the freeway might remained closed through the afternoon commute.
Southbound traffic was backed up nearly to Barstow, said CHP Officer Melanie Weaver earlier when she confirmed the freeway’s closure. Northbound lanes were open but backed up into the lower Cajon Pass due to spectator slowing.
The trouble started at 10:45 a.m. with a report of a wrong-way driver in the freeway’s southbound lanes. Six minutes later, the head-on crash was reported between Palmdale Road in downtown Victorville and Bear Valley Road several miles farther south.
The names of those killed have not been released, Weaver said. Unconfirmed reports said a third person was injured.

Man buried, dies when hillside collapses in Milpitas
When authorities arrived in a gated Milpitas hills community this weekend, they were expecting the worst: 911 dispatchers had reported a bad accident at the site where a construction crew was building a small mansion.
They found something even more gruesome than they could have imagined: a worker buried alive, crushed by an avalanche of soil. He was dead by the time rescue workers arrived, with only the top of his head visible.
Now police and state investigators are considering filing criminal charges against the contractor, who they say shouldn’t have been working at the site.
Just three days before Saturday’s deadly accident, city inspectors deemed the site was too dangerous and ordered the construction company to stop all work immediately. A combination of poor work by the contractor and the previous weekend’s rains increased the risk of the steep hillside collapsing, city officials said.
But the contractor, Fremont-based U.S.-Sino Investment, kept working on the new 5,800-square-foot house, anyway, city officials said Monday. A 37-year-old carpenter was doing wood work in a 12-foot ditch when it caved in, crushing him and killing him almost instantly, authorities said.
“It’s is one of those heartbreaking situations,” said Erika Monterroza, a spokeswoman for Cal/OSHA, the workplace safety agency that is investigating for the state.
Making this situation even more unusual, Monterroza said, was that it was initially too dangerous to remove the body. If officials jumped into the ditch, they would risk being buried, too, she said.
Late Monday evening, officials had finally pulled away the dirt and plywood and removed the body. But they have been unable to identify him.
The other half-dozen workers who were at the site at 814 Calaveras Ridge Drive were working on another part of the home when the accident occurred around10 a.m.They were all uninjured.

Many questions remained unanswered Monday.
The Milpitas police and fire departments and theSanta Clara County coroner all referred comment to the city’s building department.
A man who answered the cell phone of Richard Liu, who is listed as the CEO of U.S.-Sino Investment, declined to comment and hung up on a reporter.
The city’s chief building official, Keyvan Irannejad, said authorities won’t know until Tuesday at the earliest whether criminal charges will be filed.
Irannejad said the workers had previously cut a 12-foot-deep hole at the site to build the house’s foundation. But they didn’t put any system in place to “shore” the hole to ensure the trench would not collapse, Irannejad said.
After city building inspectors visited the site Wednesday, they issued a stop-work order until U.S.-Sino could prove they had shored the trench. Anyone found violating the order can be fined $1,000, but the city had not gone back to enforce it.
“They just ignored our stop-work notice,” he said. “When we got there (after the accident), we didn’t know they’d been working.”
Irannejad said Milpitas has never worked with U.S.-Sino before the permit was issued for the house in August. Monterroza said the state has no record of violations at the company, which began operating in 2008.
U.S.-Sino could face fines, and both police and Cal/OSHA could recommend prosecutors file criminal charges.

PERRIS: Pit bull bites 12-year-old girl
Riverside County animal control officers quarantined a pit bull that bit a 12-year-old girl this afternoon, a sheriff’s spokesman said.
The dog bit the girl near East Rider Street and Evans Road at about 3 p.m., sheriff’s spokeswoman Cpl. Courtney Donowho said by phone.
The girl was taken to the hospital for medical attention, but Donowho said she did not immediately know the extent of the girl’s injuries.

2011 statistics
- 31 U.S. fatal dog attacks occurred in 2011. Despite being regulated in Military Housing Areas and over 650 U.S. cities, pit bulls led these attacks accounting for 71% (22). Pit bulls make up less than 5% of the total U.S. dog population.2

Fontana car crash kills 3 brothers, 1 young boy

A Chevy Suburban ran a red light last night in Fontana, causing a wreck that killed all three brothers in the SUV and an 11-year-old boy in a Honda, say police and San Bernardino County coroner’s officials.
The crash was reported at 9:34 p.m. Sunday at Base Line and Cherry Avenue.
The black 1998 Chevy Suburban that was traveling east on Base Line when it ran the red light and sheared off the back of a southbound 1998 Honda containing a 40-year-old Fontana woman and the boy, police said in a written statement.
The SUV then hit a third vehicle – a Toyota – and a utility pole, police said.
Eleven-year-old Noah Ferrer of Fontana died in surgery at 12:26 a.m. today at Loma Linda University Medical Center, coroner’s officials said in a separate statement.
Two of the brothers – a 35-year-old man Rancho Cucamonga resident and a 39-year-old resident of Bloomington – died at the scene. A 37-year-old Fontana resident died soon afterward at Kaiser Foundation Medical Center in Fontana. Their names have not been released.

Woman killed in Winchester Crash
A 55-year-old Menifee woman was killed when her Toyota was struck head-on by a Pontiac on Winchester Road early this morning, the Riverside County coroner reported.
Louisa Valdovinos was pronounced dead at 4:04 a.m., 14 minutes after the crash was reported on Winchester Road just south of Scott road in Winchester, a coroner’s news release stated. A Pontiac Grand Am crossed from the opposing lane in front of her, the release stated.
The crash left two other victims in critical condition, a news release from CalFire/Riverside County fire stated. At least one of those victims had to be cut from the wreckage. A helicopter was called in to fly the victims to a hospital.
The names of those victims were not immediately available.
The crash closed Winchester Road for several hours.
The California Highway Patrol is investigating.

Man Dies After Being Struck by 2 Hit-and-Run Drivers

SANTA ANA- A crossing pedestrian was killed after being struck twice by separate vehicles on the 400 block of South Broadway Street in a hit-and-run incident late Saturday night.
The man was trying to cross the street just after 8:30 p.m. in the 400 block of Broadway when a white Chevy pickup truck struck him, sending him flying from the street’s northbound lanes into its southbound lanes. After he landed, a green Mustang covertible struck him, police said.
“Both vehicles left the scene,” said Cmdr. Larry Coleman of the Santa Ana Police Department.
The Orange County coroner’s office has not yet identified the man, who Coleman said was about 45 years old.
Police say both vehicles likely sustained damage when they struck the man.Those with information about the incident can call the Santa Ana Police Department at 714-245-8701.

Fatal Accident on 18 at Old Waterman Caynon

San Bernardino-at approximately 5:47 PM, Holly Foor, a 42 year old resident of Crestline, was driving a 2005 Honda Civic northbound on State Route 18, between upper and lower Old Waterman Canyon Rd., at mile marker 11.9, in the city of San Bernardino. A driver of a 1997 Ford Explorer who was also driving southbound on State Route 18 crossed the double yellow line into the northbound lanes and struck the Honda head-on. Foor was transported by paramedics to St. Bernardine Medical Center and died from her injuries at 6:48 PM.
San Bernardino police and firefighters responded to the two-vehicle, head-on crash, according to dispatchers with those agencies.
Police asked the California Highway Patrol to close the road shortly before 8 p.m. so they could clear the accident scene, a CHP dispatcher said, but additional information about the crash was not available.
The San Bernardino Police Department is investigating the collision.

Selma woman hurt in 3-vehicle crash
A 45-year-old Selma woman was injured in a three-vehicle crash Friday north of Kingsburg, the California Highway Patrol said.
Mary Jo Reid sustained major injuries in the collision at Mountain View and Academy avenues shortly after 8 a.m. Friday, the CHP said.
Reid was taken to Community Regional Medical Center in Fresno, where she was in serious condition today, a hospital spokeswoman said.
Reid was traveling east on Mountain View when a westbound vehicle driven by Alexandra Alves, 18, of Kingsburg, waiting to turn south on Academy, was rear-ended by a third vehicle, the CHP said. Alves’ and Reid’s vehicles collided head-on.
The driver of the vehicle that struck Alves’ vehicle was Adriana Gonzales, 25, of Dinuba, the CHP said.
Cause of the Fresno Crash
The crash remains under investigation and the California Highway Patrol is expected to release more information today.

Pedestrian Killed In Indio
A woman crossing a busy Indio street is dead after she was struck by a motorcycle and then a car.
According to Indio Police’s Ben Guitron the accident was reported just before 8 p.m. Thursday night on Indio Boulevard between Biskra Street and Arabia Street.
Police are saying a woman was walking across the street when she was struck by a motorcylcist and then two women in a car. The motorcyclist was rushed to Desert Regional Hospital, his condition is unknown at this time. The two women were only shaken up by the incident and are cooperating with investigators.
Westbound lanes of Indio Boulevard were shutdown for hours as police pieced together the accident. They have since reopened.
No word yet if alcohol or drugs were a factor in this fatal accident. The victim Brigitte Jasso 44, died at the scene.
The Riverside County Coroner was called to the accident as was the forensics department.
