Forklift accident kills Fontana man


An early morning forklift accident killed a 59-year-old Fontana man at a cement manufacturing plant in north Rialto.

Rialto Police and Fire Departments responded to the scene and found found Jack Leo Buffum, a 59 year-old resident of Fontana, had been struck by a forklift. Paramedics pronounced Buffum deceased on scene.

Jack Leo Buffum died at the scene soon after being hit by the machine at 6:53 a.m. Thursday, May 30, along the 2200 block of West Lowell Street, say San Bernardino County coroner’s officials.

Buffum was helping the operator of a 65,000-pound fork lift that’s so large that a ladder is needed to reach the cab. Together, the men were moving large concrete pipes from one side of Rialto Concrete Products to the other, said police Sgt. Tim Lane.

“The operator … lost track of the location of the assistant and ran him over,” Lane said.

Worker killed in forklift rollover

A forklift operator died Monday morning when his machine tipped over pinning him underneath, according to the Murrieta Police Department.

Antonio Toro, 38, of Riverside was pronounced dead at the scene, officials said. Murrieta Fire Chief Matt Shobert said paramedics were called to the construction site shortly after 6:30 a.m.

The Riverside County Coroner’s Office reported that the accident occurred at a construction site on Guava Street, approximately 50 feet west of Adams Avenue. A bridge is under construction in the area where the incident occurred, said Murrieta police Lt. Rob Firmes.

Toro was moving some equipment with a large industrial forklift, Firmes said, when he encountered some uneven ground and the machine began to rock over on its side. The forklift  tipped over and Toro was ejected and pinned underneath.

Truck driver killed in Chino

A 34-year-old North Carolina truck driver was killed as he attempted to unload his cargo in Chino Thursday.

Vays Dzhanayez, was delivering a load of 20-foot-long fiberglass beams to “Harrington Plastic” in Chino when the incident occurred, according to a news release from the San Bernardino County Coroner’s office.

Dzhanayez was removing straps from a 460 pound cargo when the forklift driver picked up the load and caused it to shift. The load hit Dzhanayez on the head and knocked him down. The load of beams then landed on his chest, according to the release.

Dzhanayez was taken to Chino Valley Medical Center Emergency Room where he was pronounced dead at 8:57 AM. The Chino Police Department and the District Attorney’s office are investigating.
