8-year-old riding without helmet hit in Cornelius

PORTLAND – An 8-year-old girl was airlifted to the hospital with serious injuries after she was hit by a vehicle while riding her bike in Cornelius Wednesday.

Cornelius Police Officers and witnesses said the girl was riding alone and suddenly darted out into traffic and then struck and run over by a Hummer towing a small utility trailer.

The girl was not wearing a helmet and taken to OHSU in critical condition. The driver cooperated and was not cited, police said.

Multi-vehicle crash on I-90 in Bellevue, Washington causing huge backup

BELLEVUE, Wash. – A collision involving a semi-truck, a logging truck and 13 other vehicles are blocking lanes of westbound Interstate 90 and left a young girl in critical condition.

Three lanes were closed between 148th Avenue SE and the exit to I-405 as of 10 a.m.

The Washington State Patrol said the semi-truck came upon regular morning congestion around 7:45 a.m. and could not stop in time. The logging truck fishtailed in an effort to avoid the collision. That led to a chain reaction involving other vehicles.

At least seven people were taken to the hospital, including a 9-year-old girl in critical condition.

Initial reports were that the logging truck lost its load, but that was not the case. However, there are reports the truck’s rear axle was damaged.

Two lanes, including the HOV lane, are getting through, but the backup as of 10 a.m. was approximately six miles. General traffic is being allowed to use the HOV lane. WSDOT has no estimate on when the scene will be cleared.

2 dead, 5 hurt in crash on Lake Tahoe highway

Authorities say two people are dead and five people are hurt after a three-vehicle crash on a highway on the northeast Lake Tahoe shoreline.

The Nevada Highway Patrol is investigating the crash that killed two men in Lake Tahoe.

The accident happened around 3:30 Tuesday afternoon on Highway 28 just south of Sand Harbor. Troopers say the driver of a pickup truck drifted across the median and hit a semi truck. There was a car right behind the semi, and the car’s driver pulled over to the shoulder. But the pickup slammed into the car.

Two men inside the car died at the scene. Two women who were in the car were taken to the hospital in critical condition. The driver of the pickup truck and his three passengers have non-life threatening injuries.

The accident closed that section of Highway 28 for several hours as the Nevada Highway Patrol investigated the crash.

Authorities ID man killed in WaveRunner crash at Castaic’s Pyramid Lake

CASTAIC – Officials today released the name of a 36-year-old man killed when his water scooter collided with a boat on Castaic’s Pyramid Lake.

Edgardo Almaraz, of Newark, Calif., was riding a WaveRunner that abruptly turned into the path of a 20-foot ski boat about 1:40 p.m. Saturday, said Deputy Johnie Jones of the Los Angeles County Sheriff’s Parks Bureau.

The ski boat operator went to the aid of Almaraz, who was face-down in the water, and responding deputies tried in vain for about 25 minutes to resuscitate him. Paramedics pronounced Almaraz dead at the scene at 2:22 p.m., Jones said. An autopsy is pending.

2 arrested for dog attack on California woman, 75

 

 

SAN DIEGO — Authorities have arrested the owners of two pit bulls that mauled a 75-year-old woman in a San Diego backyard, forcing the amputation of her leg below the knee.

Thirty-nine-year-old Alba Cornelio and 19-year-old Carla Cornelio were arrested Wednesday by county animal control officers. They remained jailed Thursday and could face felony charges of owning dogs that caused serious bodily injury.

Authorities say the dogs came through a hole in a fence and attacked Emako Mendoza on June 18 in the Paradise hills area. An investigation showed that the dogs attacked another neighbor on Christmas.

The dogs have been euthanized.

Inquiry: Fall River pool should not have been open

 

BOSTON (AP) – The public swimming pool where the body of a woman lay unnoticed for more than two days should never have been opened on the day she drowned because the water was too murky, state investigators said Wednesday.

The pool’s manager, its assistant manager and the regional director of the Department of Conservation and Recreation were asked to resign for their roles in keeping the state-run Fall River pool open on June 26 when 36-year-old Marie Joseph drowned accidentally. A fourth employee, the agency’s district manager with oversight of the pool, was placed on leave.

A city health inspector also has been fired.    

“We think bad decisions were made,” said Edward Lambert, commissioner of the state Department of Conservation and Recreation. “The pool should not open if there is a water clarity issue.”

State officials released the preliminary findings of an investigation into the drowning and what happened during the two days that Joseph’s body lay at the bottom of 12 feet of milky water.

Investigators said a review of surveillance video showed Joseph going down a water slide into the pool’s deep end, surfacing briefly and bumping into a child before going under. The entire sequence lasted

only six seconds. The video showed no signs of Joseph struggling, investigators said.

“Water clarity was the primary factor in preventing lifeguards from being alerted to the drowning and from subsequently detecting Ms. Joseph at the bottom of the pool after she submerged,” said Carl Rudge, chief park ranger for the agency and lead investigator.

Rudge said one of four lifeguards on duty at the time of the accident was supervising the water slide, but he noted that department rules require that two lifeguards monitor the slide and that diving blocks be closed while the slide is in use – something that also was not done.

Investigators stopped short of blaming the lifeguard near the slide, saying her attention may have been diverted by a group of other swimmers.

A short time after Joseph went under, officials closed the deep end of the pool because of the cloudy water but allowed the rest of it to remain open, another violation of protocol, investigators said.

Joseph, a native of Haiti and mother of five, worked as a hotel housekeeper in Newport, R.I. Her body did not surface until the evening of June 28, more than two days after she drowned, when youths jumping a fence for an after-hours swim discovered it.

Massachusetts pools are expected to be crowded over the next several days as a heat wave that has gripped much of the nation’s midsection moves eastward. Lambert said he was confident the state-run facilities are safe.

“This tragic event leaves heavy hearts in an agency that prides itself on its ability to provide high quality, safe, recreational opportunities,” he said.

Investigators said they were unable to corroborate a report that the boy who Joseph bumped into told two lifeguards about the incident.

The boy’s mother told the Boston Herald that her son told lifeguards that Joseph did not resurface. She also said a lifeguard told the boy that they would check, but never did.

The pool’s entire staff was placed on administrative leave after the body was found and officials closed all 24 of the state’s other deep-water swimming pools for inspection. All were later reopened except for the one in Fall River, which was drained.

Five similar water slides at other state-run pools have been closed while officials review procedures, Lambert said.

Fall River Mayor William Flanagan told The Associated Press that on Wednesday he fired a city health inspector who checked on the pool two days after the drowning, while Joseph’s body was still at the bottom and other people continued to swim in it. Flanagan said the inspector should have taken action to protect other swimmers after noting that the water was cloudy.

A second inspector who had been placed on administrative leave was reinstated after it was determined that she had not seen the water, Flanagan said.

Protocols require that the grates at the bottom of pools always be visible, but a review of the video showed the water began to cloud up on Saturday – for reasons that remain unclear – and continued to be murky for the next few days, the state investigation found.

In the future, water clarity at all pools will be tested using a 5-inch black and white disk that must be visible at the bottom of a pool before it can open, Lambert said.

Gregg Miliote, a spokesman for Bristol District Attorney Sam Sutter, said Wednesday that the DA is continuing a separate, “intensive investigation” into the circumstances of Joseph’s death, that he hopes to conclude next month. No criminal charges have been filed.

Gov. Deval Patrick ordered the state review, calling the case “terrible,” and “bizarre.”

Dr. Lauren Smith, a state public health official, said the presence of the body in the pool was unlikely to pose a health risk to other people who continued to swim.

It’s a step in the right direction and have some heads roll over this. But it still won’t bring this woman back to her family.

Motel Worker Dies After Being Pinned By Car

SAN DIEGO — El Cajon police Friday were investigating a fatal traffic accident in which a 75-year-old female motorist apparently pressed on her gas pedal by mistake and plowed into a motel maid.

 

The maid, a 60-year-old woman, was killed at Villa Embasadora Inn at 1556 E. Main St. about 12:45 p.m. Thursday, according to El Cajon city spokeswoman Monica Zech.

 

The motorist told investigators she was pulling into a parking space in front of the motel when she hit her gas pedal instead of the brake and sped forward, pinning the maid against a wall, Zech said.    
The maid, whose name was withheld pending family notification, died at the scene.

The motorist was not injured in the crash but she was transported by paramedics to a local hospital after she learned of the maid’s death and became distraught, Zech

Fontana toddler drowns in jaccuzzi

FONTANA– 20-month-old Aubrey Aguilar died at a hospital Thursday after being found submerged in an in-ground jaccuzzi at a residence.

Fontana police officers and firefighters responded to a report of the drowning just before 12:57 p.m.in the 18000 block ofFairview Drive. They arrived to find that the girl had fallen into the ground-level jaccuzzi and was found several minutes later by her parents.

The parents started giving the girl CPR as dispatchers directed them over the phone, according to a news release issued by the Fontana Police Department.

Paramedics responded and rushed the girl to Kaiser Permanente Fontana Medical Center where she was pronounced dead at 1:38 p.m.

Infant On Life Support After Car Veers onto Sidewalk

LOS ANGELES (KTLA) — A 2-month-old boy is on life support after a car veered onto a sidewalk in downtown Los Angeles and hit both a mother and the baby.

The boy, Marcello Vasquez, was rushed to a local hospital, where he was declared dead at 2:25 a.m. The mom is said to be OK.

Investigators said the baby stroller ended up pinned between one of the parking meters and the car.

The accident happened around 9:20 p.m. Thursday on Spring Street near 4th Street, shortly after an Art Walk let out.

Investigators say a man driving a new Cadillac was trying to parallel park when he mistakenly hit the gas instead of the brake and ended up on the sidewalk.
The car struck a mother and a baby in a stroller.The mother suffered serious injuries, according to police.
The baby was on life support Friday at a local hospital.Spring Street was closed in both directions while LAPD Central traffic officers investigated the incident.

Investigators say alcohol and drugs were not a factor in the accident.

The driver, who apparently was not wearing shoes at the time, is said to be cooperating with police. He was not arrested.

Police said at this point it looks to be a horrible accident.

“It doesn’t appear to be alcohol or narcotics-related,” said Capt. Todd Chamberlain of the Los Angeles Police Department. “It just appears to be a tragic incident, a slip of a foot while trying to park a car on a busy street.”

Boy Killed While Riding Go-Kart

 

A Bakersfield family is heartbroken after an accident killed their boy. 9-year-old Donald Orwig, Jr. was riding his go kart in a southwest Bakersfield alley when a truck ran over him.Family members and neighbors say kids play in the alley all the time. Tuesday afternoon began as a typical summer play day, but ended up leaving neighbors shocked and heartbroken. “He’s the greatest kid. He really loved everybody. He laughed, he played, he loved it all,” said Donald Orwig, the boy’s father. D.J. Orwig loved his electric go kart, a gift from his parents for being a good boy. “He was just amazing. I don’t know what to say, I lost my life,” said the boy’s mother, Pam Miller. D.J. was the youngest in his family, a family that is now pulling together to try and deal with their loss. “When I was looking to see who was accelerating in the alley, that’s when I saw D.J. go in front of him.” D.J.’s family says they heard the truck come around the corner, accelerate and strike the boy in the alley. “I was yelling immediately. I was going out to stop him because he wouldn’t stop right away or whatever. Anyway after I got him stopped, I ran and got my car jack and jacked up the truck best I could to pull him out,” said his father. Don Orwig, Sr. tried to revive his son with CPR. Witnesses say the driver of the pickup truck jumped out and ran, but came back with family. He does not speak English. “We were face to face and I was yelling he killed my baby and he had a look on his face like he didn’t know what I was saying,” said D.J.’s mother. The driver, 23-year-old Leonel Vasquez, cooperated with police and underwent sobriety tests. Neighbor Vicki Ware says the driver had a near-miss with her children before hitting the 9 year old boy. “We had to holler at him to stop cause there’s kids behind you, and it looked like he was kind of in a hurry.” “Speed limit in any alley way is 15 miles per hour, and basically what they’ll look at his pre- and post-collision skid,” explained Bakersfield Police Sergeant Allan Abney. Bakersfield traffic officers will look into the driver’s speed. They are also awaiting results from toxicology tests which could take four to six weeks. Ware and other neighbors are torn up, recalling what they saw. “She was hollering you killed my kid, you killed my kid come back, come back. You know just the goose bumps thinking that, that could have been my grandson.” Bakersfield police say Vasquez did not run, but fully cooperated with them. Right now he is free with no charges filed. But, if evidence shows Vasquez was under influence, he could be charged with reckless driving and involuntary manslaughter.
