Tourist helicopter crashes near Las Vegas

LAS VEGAS – A tourist helicopter based in Las Vegas crashed near the Hoover Dam on Wednesday, killing all five people aboard, the National Park Service said.
The pilot of the helicopter and all four passengers are confirmed dead, the Lake Mead National Recreational Area said on its Twitter feed.
The Eurocopter AS350 helicopter crashed during the late afternoon, according to Federal Aviation Administration spokesman Ian Gregor. The circumstances of the crash approximately 12 miles east of Las Vegas McCarran International Airport were not known, he said.
The crash occurred on the western side of the River Mountains about a half mile within the boundary of Lake Mead National Recreation Area and about 4 miles west of the Alfred Merritt Smith Water Treatment Plant, according to National Park Service spokesman Andrew Munoz.
The helicopter belonged to Sundance helicopters, according to the park service, and crashed near Lake Mead.

O.C. woman killed in Cal State Long Beach elevator accident
Tragic: Annette Lujan, 48, decided to climb out of an elevator after it got stuck between the second and third floors in an office building at Cal State Long Beach. It started moving and crushed her
A University worker was killed after being crushed to death after climbing out of a an elevator that got stuck between floors.
Annette Lujan was halfway out of the elevator when it suddenly began to move.
The 48-year-was trapped between the second and third floor as the lift began to move and suffered crush injuries.
Fire crews summoned to an administrative building at Cal State in Long Beach, California, were able to lift the 2,000 elevator off Lujan.
They administered first aid at the scene, but Lujan died later at hospital.
Fire captain Captain Rich Brandt said:’People that get stuck in elevators need to remain in the elevator.’
Officials said Lujan worked in the administrative offices of Cal State.
The accident took place shortly after workers had been arriving at the building.
Mary Stephens, the school’s vice president of administration and finance, confirmed the news in a statement to the campus.

‘I am very sad to inform you that this morning, due to an unfortunate incident at the Foundation Building, a member of our campus community has died.

She added, ‘The university has endured a terrible tragedy today. I know all of us extend our deepest condolences to the victim’s family, friends and co-workers. A counselor with the Employee Assistance Program is available for employees.
Lujan was a nine-year employee of the university’s grants department and lived in Huntington Beach
Confirmed reports claim that Lujan had alerted others that she was trapped but allegedly attempted to get out of the elevator car herself before Fire officials were on scene.
A spokesman for Cal State said: ‘This morning a Cal State Long Beach employee was fatally injured in an apparent elevator malfunction in the CSULB Foundation building.
‘Annette Lujan, who was the sole occupant of the elevator, attempted to exit the elevator after an apparent malfunction that occurred between floors.’
The statement added ‘Initial information indicates that the elevator started to move as she was attempting to exit. University Police and the Long Beach Fire Department responded immediately to a call of a woman trapped in the elevator, but Lujan succumbed to her injuries at the scene.
‘The details of the accident are under investigation. Lujan, 48, a nine-year employee a nine-year employee of the university’s grants and contracts office, was a resident of Huntington Beach.’

Worker Dies In Palm City Construction Accident

SAN DIEGO — A 58-year-old construction worker was killed Tuesday when the aerial-platform machine he was operating toppled over at an equipment yard, authorities reported.
The fatal accident occurred as the man was unloading the boom lift — a type of apparatus sometimes called a “cherry picker” — from a flatbed truck at the lot in the 800 block of Hollister Street in the Palm City neighborhood shortly before 11:30 a.m., according to the San Diego Fire-Rescue Department.
The machine tipped over and fell to the ground after one of its wheels slipped off the the edge of the truck while the worker operated it from inside its extendable basket, SDFRD spokesman Maurice Luque said.
The man, who would have turned 59 on Saturday, died at the scene. His name was withheld pending notification of his family.
The state Division of Occupational Safety and Health, or Cal/OSHA, will investigate the fatality.

APPLE VALLEY: Pedestrian badly hurt along Hwy. 18

A 35-year-old pedestrian was badly hurt Thursday night when he was hit by a motorist along a major road in Apple Valley, according to San Bernardino County sheriff’s officials.
Clifford Richardson was struck at 11:12 p.m. while walking in the traffic lanes of Highway 18 east of Apple Valley Road with his back toward traffic.
Richardson was taken to St. Mary’s Medical Center and transferred by helicopter to a trauma unit.
Seventy-year-old Floyd Keeth was driving west along the highway when he failed to see the dark-clothed victim, sheriff’s deputies said in a written statement.
Anyone with information about the accident may call deputies at 760-240-7400.

Pedestrian hit, power lines down in Ramona accident
RAMONA — A pedestrian was seriously injured Wednesday when she was struck by a car that also hit a power pole, knocking down power lines across state Route 78 and closing the highway for about an hour.
About 12:40 p.m., the driver of a Honda Civic who was eastbound on Route 78 west of Magnolia Avenue stopped to make a left turn, California Highway Patrol Officer Brian Pennings said. The driver of a Chevrolet Suburban approached the Honda from behind. The driver was not paying attention, and the Suburban collided with the rear of the Honda at about 45 mph, Pennings said.
The forced pushed the Honda into the westbound lanes, where the car struck a pedestrian, a 57-year-old woman from Ramona, who was walking east on the shoulder, Pennings said.
The Honda continued down an embankment and struck a power pole. The pole broke off, and power lines went across both lanes of the highway, Pennings said.
The pedestrian, who suffered major injuries, was taken by medical helicopter to Palomar Medical Center, Pennings said. A man who was driving the Honda suffered moderate injuries and was taken by ground ambulance to a hospital. The woman who was driving the Suburban had complaints of pain but was not taken to a hospital.

Woman trying to catch bus dies after being hit by car
Denley Angeles, 18, died just before 8 p.m. at UCI Medical Center in Orange, several hours after she was struck by a vehicle near the Katella and Harbor Boulevard intersection, according to the Orange County Coroner’s Office.
ANAHEIM – An Anaheim woman died Thursday morning after she was struck by a car while crossing Katella Avenue to catch a bus, police said.
Denley Angeles, 18, died just before 8 p.m. at UCI Medical Center in Orange, several hours after she was struck by a vehicle near the Katella and Harbor Boulevard intersection, according to the Orange County Coroner’s Office.
Officers responding to reports of a traffic accident just before 6 a.m. found Angeles in the westbound lanes of Katella, Anaheim police Sgt. Bob Dunn said.
“It appears she was getting off a bus and trying to cross the street to catch another bus when she was struck by a vehicle,” Dunn said.
The driver of the Toyota Camry that struck Angeles remained at the scene to talk to police.
Police are investigating whether Angeles or the driver had right of way. No citations or arrests have been reported.

Bicyclist injured in College Area accident
SAN DIEGO — A 23-year-old woman was injured Tuesday evening when her bicycle was struck by a vehicle in the College Area, police said.
The woman was riding south on College Avenue, near Cresita Drive, about 5:30 p.m. when she was struck by a Hyundai sport-utility vehicle driven by a 55-year-old man, said San Diego police Officer Dino Delimitros.
The bicyclist was in the slow lane when the driver attempted to pass her. There was not enough room as a vehicle was in the other lane, and the SUV clipped the bicycle, Delimitros said.
The bicycle struck the curb. The woman hit the ground and suffered major facial fractures. The driver stopped.

Pedestrian hit on Interstate 8
A pedestrian was hit Wednesday night on westbound Interstate 8 at Mission Center Road, the California Highway Patrol said.
The incident was reported about 10 p.m., according to the CHP website. A man was seen in the center median, and shortly afterward he was seen running across the freeway. The driver of the vehicle that hit him stopped in the median.
The pedestrian was transported to Scripps Mercy Hospital with life-threatening injuries, a San Diego Fire-Rescue Department dispatcher said.
Three lanes of the freeway were blocked for about 30 minutes.

Circumstances of son’s death puzzle father
Keith Epling is searching for answers about his son’s death that he knows he may never find. 
Josh Epling, 21, was hustling on foot across Interstate 15 in Lake Elsinore about 3:20 a.m. Friday, carrying a Wii bowling set purchased for his father for Christmas and a 12-pack of beer, the CHP said.
Josh Epling, moving west to east in the northbound lanes 1.5 miles south of Railroad Canyon Road, was struck in the right lane by a Toyota Scion driven by a Perris woman, according to the California Highway Patrol. The impact threw Epling into the two left lanes, where he was hit by several vehicles in the darkness. Only the woman, who was not faulted, stopped. The freeway was closed for six hours.
In the wake of the accident, Keith Epling, as well as the CHP, pondered several questions. Among them:
Why did Josh Epling scale a chain-link fence and cross the freeway, when he could have used the Olive Street undercrossing less than a quarter-mile away?
Why was he heading back to the east side of the freeway, toward the Walmart where at midnight he had bought the Wii game. He lived with his parents on the west wide of the freeway, almost three miles to the north in Lake Elsinore.
“It’s a mystery to me,” Keith Epling said softly while sitting on his porch Monday night, a candle flickering nearby.
The CHP is trying to provide some answers by reconstructing Epling’s final 24 hours, but there are some blanks.
Officer Nathan Baer said the CHP believes alcohol was a factor but won’t know for sure until it receives test results. The beer had not been opened, he said.
Keith Epling said his son had worked Thanksgiving morning installing blinds and then accompanied the family to San Diego, where it celebrated at the home of Josh’s uncle. Josh went Black Friday shopping upon the family’s return late that night, but instead of catching a ride with friends as he often did, he set out on foot.
Josh Epling was not legally allowed to drive. He was fined early this year for being a driver under age 21 with a blood-alcohol content of .01 or greater, court records show. In June, he was arrested on suspicion of reckless driving. When he later failed to appear in court, a hold was placed on his driver’s license. He later pleaded guilty to reckless driving.
Family members declined to discuss the cases.
STRONG WILLED
Certainly, Josh Epling was a determined person, his father said, and the strong will that he was born with often served him well.
He remembers that when Josh’s pickup got stuck in the beach at Lake Elsinore, Josh refused to quit on pulling it out until he succeeded – an hour and a half later.
Epling also enjoyed tearing apart and fixing his motorcycles, and doing any other kind of handiwork.
“He never gave up on anything,” his father said.
Josh was also such a voracious eater that his friends nicknamed him “Lunch Box.” After his death, they placed a potted tree on the Eplings’ porch with a blue ribbon that read “In Remembrance of Joshua ‘Lunch Box’ Epling.”
“Cleaned out my refrigerator many times,” Keith Epling said. “And he’d go out and burn the calories off as soon as he ate.” Josh loved sports, motorcycle riding and surfing, his father said.
Epling, a 2008 graduate of Lake Elsinore’s Ortega High School, had an older brother and sister and sometimes lived with a friend in San Diego.
He had several jobs in the past few years, including cleaning the horse stalls at the Del Mar race course. Before the Lake Elsinore Grand Prix this month, he helped lay out the course and then posted flags during the motorcycle and ATV races.
Keith Epling said he feels sorry for the woman who struck his son and is grateful for the outpouring of sympathy from Josh’s friends.
“I know Joshua is with Jesus Christ right now, and that’s what’s going to keep me going, my faith in Christ,” Epling said.

Pedestrian killed on Interstate in Oceanside

Michael Anthony Murgallis is dead after being struck while attempting to cross the 5 freeway in Oceanside.
Officials say that the 25 year old man was hit by multiple cars on the northbound side of the freeway near Oceanside Harbor Dr shortly before 9:30pm on Saturday, November 26.
Several people called 9-1-1 and when paramedics arrived and pronounced him dead due to obvious injuries.
It is unknown why he was attempting to cross the freeway.
CHP continues to investigate the incident.
